Yes, a plain tallow balm can go on alongside tretinoin and the other prescription retinoids, as the moisturizer step. It's there for the dryness and nothing else. On isotretinoin, the pill some people still call Accutane, the dryness reaches well past the face. A balm there is comfort for dry, tight skin.
Whoever wrote your prescription leads here, and their instructions beat anything on this page, ours included. We make the balm I'm about to recommend. For retinoid nights, I'd reach for Bare Silk ($34 as of October 5, 2026) in the Unscented original, which has no essential oils or fragrance.
Why does a retinoid dry your skin out?
The FDA approved tretinoin as a topical acne treatment back in 1971, the American Academy of Dermatology (AAD) notes. That's a long track record. The AAD credits tretinoin with spurring faster turnover of surface skin cells. Tazarotene and trifarotene are its prescription cousins, and adapalene comes both by prescription and over the counter.
MedlinePlus, the National Library of Medicine's drug guide, lists what people notice at the surface: dryness, peeling, stinging and flaky skin, along with redness and itching. It warns that acne will probably look worse for the first 7 to 10 days. Improvement takes 2 to 3 weeks, sometimes more than 6. Wind and cold, it adds, can be especially irritating to the skin you're using it on.
Rushing doesn't help. On the AAD's retinoid page, dermatologist Tina Alster says she advises patients to start with the least intense retinoid every other night, then build up slowly. Skip that, she says, and people end up irritating their skin. Sonia Badreshia-Bansal, another dermatologist quoted there, adds that on skin of color, irritation can trigger dark marks, and that starting slowly with a moisturizer helps.
Where does a balm go on a retinoid night?
MedlinePlus says to wash with a mild, bland soap and let your skin dry as directed. The tretinoin goes on at bedtime, in a thin layer. "As directed" means the wait between washing and the retinoid is your prescriber's number, and I won't invent one.
A balm can sit in two places. After is the simple one: the retinoid goes on dry skin, and once it's settled, a pea of balm warmed between your fingertips gets pressed over it. Before is the buffer, a thin layer of balm under the retinoid. Do both and you've built what people online call the sandwich, a retinoid between two thin layers of balm.
The AAD says acne treatments like tretinoin tend to dry and irritate the skin, and its dermatologists recommend a moisturizer every day your skin feels dry. MedlinePlus says your doctor may tell you to use one. None of these pages ranks one order over another, and I haven't seen a study that settles it. Use the order your prescriber gave you.
On nights off and in the morning, the balm goes on slightly damp skin instead. That's where occlusives work best, according to a 2016 review of moisturizers by Sethi and colleagues in the Indian Journal of Dermatology. The AAD's acne page agrees: leave some water on your face after washing, then moisturize.
The reason for the prescription matters too. If your tretinoin is for breakouts, the AAD asks for a moisturizer labeled oil-free or non-comedogenic on acne-prone skin. A tallow balm isn't oil-free, so ask before it goes on breakout-prone skin. Whether tallow clogs pores is its own question, and our post on whether tallow is comedogenic works through the evidence.
What doesn't a balm change?
A balm on top doesn't license using more retinoid, or using it more often. MedlinePlus says to use tretinoin exactly as directed, and to keep it off sunburned skin and away from your eyes and mouth. Cracked or bleeding skin is broken skin, and the balm stays off it too.
The same page says preparations heavy in alcohol, menthol, spices or lime can sting, especially when you're new to tretinoin. Perfumes are among its examples. That's one reason the jar I'd hand you on a retinoid has no essential oils or fragrance at all.
Mornings change too. Retinoids can make skin more sensitive to the sun, the AAD says, so Alster advises using them only at night, with sun protection every day. Tallow doesn't screen UV, so it can't stand in for sunscreen. Sunscreen goes on last, over the balm, and for sensitive skin the AAD suggests a mineral one with titanium dioxide or zinc oxide.
What changes on Accutane?
Isotretinoin is the pill, sold by prescription only for severe acne. The AAD notes that some people still call it Accutane, a brand that's no longer available. A course takes about four to five months. It sits inside a program called iPLEDGE, which has rules of its own.
The AAD calls dry skin and chapped lips common on isotretinoin, along with irritated eyes and even nosebleeds. The dryness clears once you stop, it says. For acne-prone skin like the face, neck, shoulders and back, it suggests an oil-free moisturizer or an emollient, applied while skin's still damp after cleansing. After bathing, it suggests a rich, creamy moisturizer or emollient. Either way, it asks for products labeled hypoallergenic or non-comedogenic, which is worth checking on any jar you use, ours included.
A tallow balm counts as an emollient in the AAD's sense, softening and smoothing, which makes it a plain fit for dry hands and shins after a shower. For lips, the AAD names petroleum jelly through the day and before bed, and an SPF 30 or higher lip balm outdoors. Petrolatum at 5% or more cuts water loss by more than 98%, Sethi's review reports. For lips on isotretinoin, I'd follow the AAD. For everyday chapped lips off the drug, our post on tallow for lips has the method.
Which jar, and which one sits out?
It's cosmetic care, not a treatment, and your provider's plan comes first. For a dry face on a retinoid, and for dry skin on the body during isotretinoin, I'd hand you Bare Silk Unscented. It smells like nothing. A pea, warmed between your fingertips and pressed on, is enough for the whole face. Velvet Face Renewal ($89 on the same date) sits this one out while you're on a prescription retinoid, unless your prescriber okays it. It carries 1% bakuchiol. That's an ingredient tested head to head against retinol for the look of fine lines, so stacking it on a prescription retinoid doubles up on the job. Bakuchiol vs retinol walks through the trial.
Start with Bare Silk ($34), five ingredients and sixty days to disagree.
Questions we hear
Can you use tallow with tretinoin?
Yes, as the moisturizer step on intact skin, in the order your prescriber sets. Tretinoin goes on at bedtime, on skin you've let dry as directed, and a pea of plain, unscented balm can follow it as a seal. A thin layer can also go under it as a buffer. If the tretinoin is for breakouts, ask about those areas first, because the AAD's acne advice asks for moisturizers labeled oil-free or non-comedogenic.
Should moisturizer go on before or after a retinoid?
Both orders are in use, and none of the sources I read ranks one over the other. A moisturizer after the retinoid seals the surface. A thin layer before it acts as a buffer, and doing both is what's called the sandwich. The AAD says treatments like tretinoin tend to dry and irritate skin. MedlinePlus says your doctor may suggest a moisturizer for dryness, so follow the order your prescriber gives you.
What helps dry skin on Accutane?
The AAD's tips for isotretinoin start with an oil-free moisturizer or an emollient on acne-prone skin like the face and back, applied while skin's still damp after cleansing. After bathing, it suggests a rich, creamy moisturizer or emollient. For lips, it names petroleum jelly through the day and before bed, plus an SPF 30 or higher lip balm outdoors. The dryness clears once you stop, the AAD says.
Should you start a retinol product if your skin is already on the dry side?
The AAD says people with skin allergies or dryness probably aren't good candidates for a product with a retinoid in it. A dermatologist it quotes advises starting slow, every other night. Over-the-counter labels don't have to state the retinol percentage, the AAD adds, so a board-certified dermatologist can help you pick.
